The CARE Network Disease Management program targets patients with potentially high-cost chronic disease states. Our target diseases include but are not limited to: Asthma, COPD, CHF and Cardiac related diseases, Depression, and Diabetes. Our Disease Management staff are clinicians with expertise in these areas and will coordinate the clinical needs of the patient. This includes education about the disease, self-monitoring, importance of medication compliance, medical resource access, community information, etc.
